offers a limited benefit "inpatient
facility only" individual health
insurance plan. This plan provides
health insurance coverage for those
situations where inpatient care is
necessary. In-Network Foundation
Hospital Care pays 80% and you pay
20% of covered expenses after you
meet your calendar year deductible.
Emergency Room visit is subject to a
$200 co-payment amount and calendar
year deductible, then pays 80% and
you pay 20%. Foundation Hospital
Care also offers affordable,
age-banded rates for child-only
coverage With Foundation Hospital
Care, premiums for child-only
coverage are based on the age of
your child, not whether you purchase
other coverage. One application per
