maximum, preventative care, prescription
drug program and emergency room benefits. A

options in design. The PPO network of
doctors and hospitals is one of the largest
in the country. This is an 80%/60% plan,
with options for a $0 deductible
prescription plan and an option for an
office visit copay.
When you add the DENTAL option, it gives you
a 100% benefit for preventative services,
50% for Basic and Major services with a $50
deductible and an annual maximum benefit of
