of doctors and hospitals is one of

an 80%/60% plan, with options for a
$0 deductible prescription plan and
an option for an office visit copay.
This option, the Office Visit Copay,
allows you to bypass the deductible
and pay a $25 copay in a primary
doctors office and a $40 copay in a
